- Prior consent for joint use of administrative information (*completed attachment)

- One copy of resident registration and one copy of resident registration (*submitted only if they disagree with the joint use of administrative information)

- One photo (half name plate) and one copy of the bankbook (payment account)

- In the case of persons with disabilities, persons of national merit, and persons eligible for medical care, one copy of the relevant documents (limited to those concerned)
