- Prior consent form for joint use of administrative information (*Preparation of attachment)

- One copy of each resident registration and one copy (*Submitted only if you do not agree to the joint use of administrative information)

- One photo (return box) and one copy of the bankbook (account for salary payment)

- In the case of persons with disabilities, persons of national merit, and persons eligible for medical benefits, one copy of the relevant documents (limited to those concerned)
